          It was a routine evening OPD on 29/11/2019 at Vatsal Eye Care. 35 year old farmer comes from Bhuigaon village to get his eyes checked. Since two months he was seeing “floaters” ie something was coming in front of his right eye. Initially he was ignoring his symptoms. But then he constantly felt as if a shadow was coming in front of his eyes, felt like a worm in the eye and was even hampering his vision. He kept telling his friend in the same village. They both searched on Google for Eye Doctor in Vasai and took an appointment at Vatsal Eye Care. When he arrived his right eye was slightly red and painful.
            
          Dr Pritam Dedhia examined his retina and confirmed that this something which was moving was definitely a “live worm” in his eyes and had to be surgically removed. Diagnosis was confirmed – Ocular cysticercosis. This was a very rare case but since Dr Pritam has handled such cases before, he was pretty confident to tackle it. Everything about his condition, surgical treatment and post operation care was explained to the patient. In order to rule out Neurocysticercosis, MRI of brain was done and it was normal.

OTHER TREATMENT OPTIONS: 
Patient  was given the option of treating it with Tablet Albendazole and killing the worm inside, but there is a chance of toxic effects on retina and macula or the worm migrating somewhere else. He got assured he is in safe hands and decided to go for the surgery.

WORM IN EYE SYMPTOMS:
            Eye worms in human is a rare condition. According to WebMD “Eye floaters appear as small spots that drift through your field of vision. They may stand out when you look at something bright, like white paper or a blue sky. They might annoy you, but they shouldn’t interfere with your sight.”  
           
Other causes of floaters are age related eye changes, inflammation in the back of the eye, blood in the eye, retinal tears, diabetic retinopathy, eye tumors. Consult the eye specialist when floaters are increasing, darkness in the vision or onset of new floaters.

            Cysticercosis is a parasitic infection of the eye caused by the larva of Taenia Solium (pork tapeworm).


LIFE CYCLE OF TAENIA SOLIUM
Sources of infection:
1) Eating food and drinking water which    is contaminated with the eggs of Taenia Solium.

2) Eating raw or uncooked pork.

3) Poor hygiene

4) Autoinfection



 SURGICAL TREATMENT OF “ EYE WORM” AT VATSAL EYE CARE:
            
Peripheral Vitrectomy was done and the worm alongwith the cyst was removed. Do watch this beautifully captured surgical video of this rare case in ophthalmology.


The worm was sent for Histopathology testing which confirmed that it was Taenia Solium. Hence the clinical judgement was correct. Two weeks after the surgery, floaters had disappeared and patient also had significant improvement in his vision.
